Registration & Donations
Samuel Waxman Cancer Research Foundation requests certain information from you on our SWCRF Donation and Events pages. To donate to SWCRF or to purchase tickets to an event, you must provide contact information (i.e., name and billing address) and financial information (i.e., credit card number and expiration date). This information is used for billing purposes and to fill orders. If SWCRF has trouble processing a donation, SWCRF may use this information to contact you.
On our financial transactions pages, SWCRF collects the following personal identification information: name, address, email address, phone number and credit card information. This personal identifiable information you submit is used to acknowledge receipt of your donation for tax purposes and to include you on the SWCRF mailing list. Samuel Waxman Cancer Research Foundation online donation page has an “opt-in” entry for those who want to donate but do not want to be added to our mailing list.
In addition, when you send a donation in honor or in memory of someone and request an acknowledgment be sent to another individual, we will use the name and address of that individual for acknowledgment purposes only.
